Host Home Provider Program Overview

Through the Vital Choice, Inc. (VCI) residential program, people receiving services may live with a host family or a single individual of their choosing who best provides the support that meets their needs. There are a variety of different host homes and depending on what the person is looking for in a particular setting, VCI applies a detailed matching process to connect the person with potential host home providers.

What is a Host Home Provider?

Host Home Providers (HHP) open their homes to provide a loving, caring, safe family environment with 24/7 support to a person receiving waiver services. Host Home Providers provide supervision and life skill-building opportunities and provide personal care

Your Part of a Team

The Host Home Provider is part of a larger Interdisciplinary Team (IDT). The IDT is comprised of family members, other service providers, advocates, and case managers. They all have a strong interest in the well-being of the person receiving services.  Collaboratively, the IDT advocates for the interests, of the people living in HHP’s home.

The Pay is Great!

Host Home Providers are paid very well. Depending on the Level of Care the client requires, fees are proportionate and are paid on a standard percentage basis.

To ensure you succeed, you’ll have the support of our dedicated Vital Choice team every step of the way, but we also offer something no one else does – our exclusive Host Home Mentor Service. You’ll have access to a mentor who is committed to your success. With us, you’re not just joining a program; you’re joining a community.
