Direct Support Staff

Our Direct Support Professionals (DSPs) team of dedicated and well-trained staff is committed to delivering individualized care tailored to the specific needs and preferences of each resident. Our staff assists with daily living activities offering hands-on support with tasks like bathing, dressing, housekeeping, laundry, meal prep, problem-solving, emotional support, general life skills, and safety monitoring.
Live with a caregiver
Host Home
Live with a caregiver in their home: In a Host Home, 1-2 individuals with developmental disabilities live with a trained caregiver in a family-like setting. Host Homes are often staffed by a single provider whose family may also reside in the home. Host Home Providers (HHPs) offer support with daily activities and community involvement. Most often, the HHP provides all the care inside the home.

PCA Independent Living
Live independently with regular support service visits and staff on call 24/7. Our PCA Services and staff come to your home, condo, or apartment to assist and support you with independent living skills and homecare support on a scheduled or as-needed basis. Staff are on call 24/7 for emergencies.

PCA Small Group Living with 24/7 Staff in the Home:
Our PCA Home is designed for 3 individuals who live in a shared state-of-the-art home. Multiple staff work around the clock on rotating shifts. Our PCA home offers private bedrooms, communal living spaces, and a variety of staff-supported activities, meals, and outings, ensuring a close-knit, supportive community. Vital Choice’s PCA Home has a cozy atmosphere where residents can feel relaxed and secure with staff always present and ready to help you.

Live independently
What is the Family Caregiver Model?
Through the Family Caregiver model, family members can become paid caregivers, directly delivering essential services like personal care, medication management, skill development and community integration. The caregiver is hired and supported by an authorized IDD Services Agency including Vital Choice, Inc. As a family caregiver, you will be considered as a contractor of our agency, which supports your training, compensation and adherence to state regulations.
Respite Services
Vital Choice offers Respite Services to provide temporary relief or fill-in support to caregivers while ensuring individuals receive the care they need. Our program offers short-term care in carefully selected host homes and PCA model homes, allowing primary caregivers to take a break and recharge.
